Cooper Rebecca

I have worked as a pharmacist for more than 25 years. My role at Saint Thomas Medical Partners Family Health Center - West is clinical. Patients are referred to me by providers for medical education and support, initiation and titration of medications, medication reconciliation and education on behavior changes to positively affect health outcomes and lifestyle choices. Helping patients afford medications by providing information on Patient Assistance Programs and changing meds based on insurance formularies or obtaining prior authorizations are daily activities.

I am part of our clinic's leadership team in addition to my clinical role. In the leadership side of my role, I help in the vetting process of new providers as well as order medications for clinic use and serve as our back-up Vaccine for Children's coordinator, as well as provide senior leadership with financial reports related to the community benefit side of our clinic. I have a collaborative agreement with the clinic's Medical Director to authorize prescription refills and make medication changes in existing therapy and regularly consult with prescribers to advise them on evidence-based medicine practices and the most appropriate prescribing patterns for individual patients. Along with our Medical Director, I provide educational talks and updates to our Nurse Practitioners on at least a quarterly basis.

Beyond my role at Saint Thomas, I have given talks on medication use at churches, senior citizen's centers and participated in career days at various schools.I have worked as a pharmacist for more than 25 years.
